Program Purpose: Provide an eye exam and glasses to low income residents of the Ardmore, Oklahoma area who have no other resources available.

To apply for financial assistance in obtaining eyeglasses, complete the attached form and mail to the Ardmore Lions Club. Your application will be reviewed by the Ardmore Lions Club Eyeglass Committee. The committee will contact the applicant concerning approval / disapproval.

You must live in the Ardmore, Oklahoma area for your application to be considered.
